Client intake forms are a necessity for performing services. As these may take a while to fill out, please allow 15 minutes prior arrival time for this process.You may also stop in to pick the forms up prior to your massage sessions date should you want to fill them out prior to your arrival date.

In addition there are medical contraindications that sometimes arise.

If you are currently under a physician's care for a medical condition, please stop in or call so that the proper release form may be given to you for the doctor's consent.

Additionally, I also have a form that is for a physician's referral for massage.

This provides both the practitioner documentation, but also provides you (the client) the ability to have it possibly written off as a medical expense yearly with your tax consultant.
